Osäker vård? En fallstudie inom svensk hälso- och sjukvård

The swedish healthcare is in many situations one of the best in the world. But still, the healthcare system includes elemnts that reduce the efficiency. Partly becouse the swedish healthcare is largely monopolized, the servicecs are not properly prized and the economic responsibility and the liability legislation is limited. The main problem this study evolves around, is how a different liability rule would change the shape of the healthcare organisation. Abouve all, how would the approach to riskmanagement and preventive and riskreducing performance change with a diffrent liability rule.

Valfrihet inom den svenska hälso- och sjukvården: En studie av patienters beslutsfattande

Citizens? freedom of choice has been a subject of debate in Sweden for over 30 years. This study focuses on decision making processes among patients within the Swedish healthcare system. The main conclusions are that in contrast to how patients? freedom of choice is described within governmental bills and governmental inquires and reports, patients are not following the steps included in the rational model of decision making.

Swedish healthcare systems have endured harsh criticism from healthcare professionals because of their inability to understand healthcare processes. These systems have been identified as significant time consumers, and Swedish healthcare is therefore facing a major challenge to assure patient safety. The following paper is based on a workplace study, which aims to examine healthcare systems? impact on physicians? practices. Our findings show that physicians spend substantial amounts of time chasing information, rather than managing information and pursuing healthcare.

Jämlikhet i vården : - en kritisk ideologianalys av privatiseringens inverkan på betydelsen av en jämlik vård

This thesis investigates if privatization of the Swedish healthcare system influences our view on the meaning of equality. This system is based on the principle that all people are equals and that healthcare should be provided on the basis of need. I apply critical ideology analysis to investigate how our view of equality might change because of the evolvement of society. A critical view may be explained as an interest in exploring things that seem obvious to many. Important questions to answer are why equality is regarded as such a natural concept in the society and how healthcare is actually provided.

Drömjournalen : En studie av vårdmedarbetares upplevelser av samtida och framtida vårdkommunikation

The effectiveness of communication in the Swedish healthcare system context has long been the subject of debate. Issues stemming from usability, accessability and interpretation affect the lives of patients, and yet the vocal groups in the healthcare communication debate are patients and administrators, not healthcare professionals. With the advent of eHealth services and new demands being put forth on the ability of healthcare professionals' to handle increasingly numerous and innovative systems, it is of more importance than ever to understand the healthcare professionals' work context. This article is based on interviews with five healthcare professionals concerning their views on electronic health records (EHR), eHealth services, trends in healthcare services development in Sweden, as well as their views on the future of eHealth solutions and health information technology (HIT). Analysis of collected information shows an apprehension among healthcare professionals towards electronic communication, as well as a dissatisfaction with the usability of modern healthcare systems.

Requirement Specification for Information Security to Health Systems, Case Study : IMIS

During 2001-2002 a prototype, IMIS (Integrated Mobile Information System) was developed at BTH (Blekinge University of Technology) to demonstrate how mobile IT-systems can be used in healthcare. The prototype was based on the activity theory of Engeström. An ongoing project started in spring 2003. The purpose of the project is further development of IMIS with special focus in the diabetes healthcare. Participants in the project are scientists and students at BTH, ALMI Företagspartner, Blekinge FoU-enhet, Barndiabetesförbundet Blekinge, Blekinge Diabetesförening, Vårdcentralen Ronneby and Vårdcentralen Sölvesborg.

Är handdatorer säkra nog för hälso- och sjukvården?

The aim of this report is to discuss computer security in handheld computers and to find out the appropriate security level that must be implemented to be able to use handheld computers within the Swedish healthcare system. Most healthcare centers are using some kind of electronic journals for their patient´s data today, but there are still a number of clinics that are usingthe old paper-system. On a few places in Sweden the use of handheld computers at the clinics has already started, but in the United States the use of handheld computers in the medical area is already widespread and from this we can see a lot of different situations where it is useful and effective to work with handheld computers. The security demands on patient´s journals are high and regulated in both Swedish law and regulations from the National Swedish Board of Health and Welfare. The law does not have any substantial demands, but states that there should be an adequate security level based on the nature of the information and the costs.

Motivation till förändring : hur sjukhuspersonal kan motiveras till att arbeta med Lean Healthcare

The Swedish healthcare system is characterized by lack of resources and extensive hospital queues. In order to counter these difficulties the Lund University Hospital have introduced Lean Healthcare to the hospital organization. The purpose of this scientific paper is to study the relationship between the implementation of Lean Healthcare and the experienced motivation among the staff. This essay thereby intends to impart knowledge, which could be used on future implementations of Lean Healthcare at other hospitals. In order to fulfil the purpose of this essay a case study have been conducted at two wards at the Lund University Hospital in which qualitative research interviews have been the method chosen in order to gather data.

Användning av IT som resurs i sjukvården : En studie om interoperabilitet i vårdens eHälsosystem

Today significant resources are provided to improve the digital work environment within the Swedish healthcare to create efficiency, good work environment and patient safety. Despite this, major flaws are shown by the lack of communication between the eHealthcare systems. This becomes a stress factor among healthcare professionals because of increased administration and decreased physical contact with patients. The purpose of this study is to create a understanding of the concept of interoperability at different levels in the eHealthcare systems. This has been conducted through a qualitative interview study at a healthcare center where the respondents have been describing their digital work environment and the way it works today.

Att öppna upp sjukvårdens förändrings- och utvecklingsarbete : En studie av medskapande innovation i primärvården

Healthcare in Sweden are in need of a drastic change to meet the requirements of highly developed societies, and information technology is seen as an essential part to get there. However, the Swedish healthcare have to challenge their traditional innovation approaches as well. For example, it has become more common to engage patients in the innovation process through web-based platforms. We have been focusing on an approach called co-creation by studying a workshop, in which potential users and other stakeholders are cooperating creatively to come up with innovative solutions in order to improve healthcare practice. Our findings suggests that co-creation may increase the mutual understanding between different stakeholders, for example healthcare professionals and IT developers, enable horizontal learning through and outside the organization and also to raise concepts and actions that are responding to healthcare needs.

Uppföljning inom ett vårdvalssystem - En fallstudie av uppföljningen av husläkaruppdraget i Stockholms Läns Landsting

This study aims to examine the effect the evaluation system used by the Stockholm County Council, has on how the healthcare providers themselves conduct the evaluation regarding their own operations. This is done through a qualitative case study of the evaluation systems of the Stockholm County Council and of four primary healthcare producers concerning one primary healthcare contract. The evaluation systems were analysed using Merchant and Ver der Stede's (2007) framework for different types of control. In order to give a more detailed picture this framework was complemented with different types of performance measurement. The evaluation that the Stockholm County Council conducts was found to have little influence on the providers own evaluation with the exception of the quality indicators that are rewarded with a monetary bonus..

Kan Lean Healthcare vara den bit som saknas i den svenska sjukvårdens pussel? : En studie av kommersiella modeller i svensk sjukvård

BakgrundSvensk sjukvård har idag bland de bästa behandlingsresultaten i Europa, men trots det förs diskussionen om sjukvården ofta i negativa ordalag. I ett försök att förbättra synen på verksamheten och bli mer effektiva har sjukvården i Sverige ofta använt sig av kommersiellamodeller, modellerna har dock behövt anpassas för att fungera i sjukvårdens miljö. En av de senaste kommersiella modellerna som nu införs i den svenska sjukvården är Lean Healthcare,som är sjukvårdens version av Toyotas produktionsfilosofi Lean Production. En stor del avtidigare forskning på området har behandlat Lean Production men lite finns att tillgå som handlar om Lean Healthcare. Den här studien är ett bidrag till att utöka kunskapen om Lean Healthcare i Sverige.SyfteSyftet med den här studien är att beskriva hur Lean-filosofin fungerar i den svenska sjukvården samt att göra en jämförelse mellan Lean Production och andra kommersiella modeller som tidigare har införts i den svenska sjukvården.GenomförandeMed hjälp av intervjuer på tre svenska sjukvårdsenheter har en fallstudie med fokus på Lean Production och Lean Healthcare genomförts.ResultatResultatet av studien visar att Lean Healthcare, i likhet med tidigare modeller, har krävt anpassningar för att fungera i sjukvårdens kontext.

Lean Healthcare på USiL - En förändring mot lärande

I sin strävan att bli en lärande organisation har USiL stött på problem och motstånd. Tidigare misslyckade förändringsprojekt har bidragit till ett förändringsmotstånd bland de anställda. Motståndet stärks ytterligare av den kultur och struktur som finns på sjukhuset. Lean Healthcare innebär nya roller för de anställda. De grupper som kan anses vara förlorare med ett införande av Lean Healthcare är chefer och läkare eftersom de tappar makt och inflytande.
